Desmopressin is a drug used in the treatment of urinary continence problems in MS. It is specifically used with people who experience nocturia - the need to go to the toilet frequently during the night. The drug can also be used during the day if regular access to a toilet is not convenient, for instance during long haul travelling. However, it is essential that the user recognises the possible dangers of retaining too much water if it is used more than once in 24 hours.
How desmopressin works
Desmopressin is synthetic antidiuretic hormone, which means that it suppresses the production of urine by the kidneys. It can reduce urine production for six to eight hours.
How is desmopressin given?
Desmopressin is usually taken orally as a tablet. Only one dose should be taken per day, with no fluid for one hour before and eight hours after use.
A nasal spray (product name Desmospray) is now used less frequently due to concerns about the effect on an individual's sodium levels.
Side effects and contraindications
People over 65 or those with diabetes, heart or kidney problems should not be prescribed desmopressin, nor should people with dependent oedema (an increase in fluid in the body, which can be caused by inactivity or reduced mobility).
Rarely desmopressin tablets can cause a lowering of sodium levels in the blood (hyponatremia). The symptoms of this are nausea, headache and generally feeling unwell.
Desmopressin should not be taken in conjunction with diuretic or blood pressure medication.
